REQUEST FORMS
FOR CBC STAFF USE ONLY
If you are having an event that needs to be communicated to the church as a whole or you have design needs, please fill out the Communication Request Form (this doubles as a Facilities Request Form).
Please fill out the Facilities Request form if you are only needing to reserve CBC Facilities and don't need any external communication for your meeting/event (i.e. Leader Meeting, Conference Room Reservation, etc.).
DO NOT use this form if you are wanting communication via announcements, social media, etc.
Please complete this form for any personal conference, seminar, class or training you plan to attend at the expense of CBC. Do not register or book travel until you receive final approval for the training from your department supervisor.
